This is the first form a pilot needs to fill out when expecting to be out over 2 weeks. It is a simple one page form. It must be submitted to the company within 22 days of calling in sick to be paid sick time. No medical information is shared with the company on this form.
Our LTD plan requires a pilot to use all the sick time available during the elimination period unless the pilot has more than 195 hours. If a pilot has more than 195 sick hours available when initially calling in sick for a disability, the pilot can use this form to keep some sick time for returning to the line. This form must be submitted to the flight office within 60 days.
This form allows a pilot to use as little as 65 hours per month up to as much as 130 hours per month during the elimination period. If this form is not completed, the company will pay the pilot 85 hours per month until sick time is exhausted.

If you are enrolled in Pilot Mutual Aid or Pilot Occupational Disability (formerly Loss of License) with APA, this form must be completed. The same form is used for both plans. If you have both plans, the form only needs to be completed once.

The Employee Statement and Authorization to Obtain Information must be filled out by the pilot.
Tell your flight office to fill out the Employer Statement. They have these so no need for you to provide it to them. The flight office will send it directly to Harvey Watt and it will meet up with your file.
take the Initial Physician Statement to your treating doctor. Have them fill it out and give back to you. Send it in with your Employee Statement and Authorization to Obtain Information.
